There are two options to schedule your scan. The web form below, or by calling our offices directly.

Web Form

Please complete the form below and submit. Please note that this is a request for a scan that will be sent directly to our scheduler staff members. Please ensure to include a phone number we can reach you on.

In many cases, we should be able to accommodate the day and time requested, or close to it, however, this has to be verified since the schedule can change quickly, and in some instances, the day requested will need to be adjusted if an insurance authorization is required.

A staff member will contact you to verify the day and time. Please do not assume the scan request is final until confirmed by a Partners staff member.

Please note: X-ray scans are on a walk in basis. No scheduling is required. First patient times are 9am Monday to Friday, and last patient walk-in is at 5pm.
